Anesthesia

Anaesthesiologists are physicians trained in anaesthesia and perioperative medicine or better known as “perioperative physicians”. They are specialist doctors involved in preoperative evaluation and optimization of patients,  ensuring intraoperative life support and perioperative pain and fluid management and ensuring quality recovery. In addition,  they are actively involved in patient care in intensive care units and emergency airway and venous access,  particularly in trauma and extremely sick patients in Emergency department. Our department has been providing successful painless labour services in the form of epidural analgesia since 1994 on a regular basis. Management of acute and chronic pain is also undertaken through pain and palliative care unit.

 

DEPARTMENT HISTORY

Since its inception in 1970, Dr Elizabeth, Dr Thomas Sebastian, Dr Mohan Mathew and   Dr Shaloo Ipe (since 1983) guided the departmental activities.
The institution was promoted to the status of a teaching hospital in 2002 and Dr. Saramma P Abraham joined as Professor and Head of the Department.
Post graduate training in Anaesthesiology started in 2011.


ACADEMICS

UG Teaching programme: MBBS students are posted for 15 days for clinical training in the department of Anaesthesiology during their surgery postings. During their postings in the department,  besides clinical exposure , they have demonstration of anaesthetic techniques and resuscitation training conducted by the faculty. Lectures (including palliative care classes) are scheduled during sixth semester.


PG Teaching Programme - M.D (1seat/year) & D.A (2seats/year): Post graduates are rotated between various surgical specialties. Faculty members take introductory lectures on induction of these students and guide them step by step during their postings in the operation theatres,  post operative care room and in the Intensive Care Units (ICU). The academic programme consists of lectures, discussions,  seminars,  case presentations,  journal clubs and periodical written tests. They are encouraged in research work and in attending CMEs,  clinical society meetings etc. For the benefit of examination going postgraduate students,  pro con sessions,  mock vivas and rapid fire discussions are held. Journal club meetings are held fortnightly.

PG teaching schedule:

  • Lectures (Faculty) - Friday 2 - 4 pm
  • Case presentation/ discussions - Tuesday and Thursday 2 - 4 pm

Diploma in Anaesthesia Technology and Critical Care (CMAI): This is a longstanding philanthropic effort by the department to train paramedical students of the local community to be a valuable part of the larger medical fraternity and aspire to be the best in their field of service. The department has been facilitating and supervising hospitals all over India,  who step forward to socially uplift the community around them,  by providing certified training and facilitating placement. This course is affiliated to Christian Medical Association of India (CMAI).

  • Comparison of dexmedetomidine and fentanyl as adjuvants to ropivacaine in lower abdominal surgeries – an observational study.
  • Dexmedetomidine as an adjuvant to ropivacaine in ultrasound guided supraclavicular brachial plexus block – a double blinded,  randomized control trial.
  • Comparison of efficacy of Dexmedetomidine and Fentanyl in reducing stress response during extubation in Tympanoplasty patients - A double blinded,  prospective,  randomized control trial.
  • Incidence of hyponatremia after TURP surgery in diabetic patients in comparison with non diabetic patients- a prospective observational study.
  • Incidence and risk factors of immediate severe post operative pain following laparoscopic sterilization under general anaesthesia – a cross sectional study” from January 2016.

Anaesthesia services extend from the newborn to geriatric age groups. Procedures like Video Assisted Thoracoscopy Surgery (VATS) for Congenital Diaphragmatic Hernia are done in newborns by our paediatric surgery team. We provide anaesthesia for General surgery,  Obstetrics and Gynaecology,  ENT,  Orthopaedics,  Ophthalmology and super specialities like Neurosurgery, Cardiothoracic surgery, Paediatric Surgery,  Plastic Surgery and Urological surgeries. Anaesthesia for interventional neuro procedures like coiling for aneurysm,  AV malformations and cardiac procedures like coronary angioplasty etc are regularly being carried out in the cathlab.

Portable Ultrasound and Fibreoptic bronchoscope
Portable ultrasound is utilized for ultrasound guided nerve blocks,  vascular access like central venous catheterisation and arterial cannulation.
Fibreoptic bronchoscope is routinely used for difficult airway access.

These procedures are done at the bedside in our ICU as a routine. These services and ventilator management by the anaesthesiologist are extended to other ICUs also on request.

Specialty clinics and services being provided by the department

 

  • Pain clinic and acute pain management

Pain clinic OP is run on a regular basis on Wednesdays and Saturdays with round the clock services offered by the duty Medical Officer. Patients are also seen in wards on a cross consultation basis. Treatment modalities include conservative measures including oral opioids and invasive measures like specific nerve blocks when necessary.
The department also manages acute post-operative pain and that of trauma victims (e.g. fracture ribs) by modalities including parenteral medication, central neuraxial blocks and nerve blocks.
